In an effort to maintain a relaxing atmosphere, infants and children under thirteen (13) years of age are not allowed in the office unless they are being provided a service.
A Parent or Legal Guardian must check in all children under age sixteen (16).
If you are more than 15 minutes late for your appointment, your office appointment may have to be rescheduled. Please take the time to call ahead.
Our office is a Non-Smoking and No Food allowed Facility.
Please notify the staff if you are PREGNANT OR THINK YOU ARE PREGNANT.
Please take the time to update us each visit of any significant Medical Problems or Medications that may have a negative impact on your scheduled service.
No EXCEPTIONALLY LOUD TALKING or VULGAR LANGUAGE is permitted in the office.
A 24 hour Cancellation Notice is Expected.
In order to maintain a Quiet and Relaxing Environment, these Policies will be Strictly Enforced.
NO CHECKS ACCEPTED.
We accept Cash, Money Orders, MasterCard, Visa, Discover, American Express and Debit Cards.
No Credit Accounts will be established in our office. Payment will be due when services are completed.
Our office will bill approved your Insurance Company for approved services. Please contact your company in 14 days to assure that your claim has been paid.
